MUTT KUTTZ DOGGIE SPA GROOMING RELEASE FORM

 

**PLEASE READ CAREFULLY AND SIGN**

 

**Mutt Kuttz Doggie Spa will be referred to Mutt Kuttz throughout the document. Owner is understood to refer to pet owner and/or all responsible for the pet and payment of said pet**

 

MATTED PET(S): PLEASE SEE our Matted Pet Release Form, both forms must be filled out on matted pets. Mutt Kuttz employs extensive safety and sanitary precautions to ensure your pets safety and health. Despite reasonable efforts, some pets may experience common side effects of the grooming process, including but not limited to: clipper irritation (otherwise referred to as clipper burn) exposure of the quick, reactions to flea treatments and deshedding treatments, hair splinters, and/or sharp nails after clipping. By signing this agreement, you, the client, agree to relieve Mutt Kuttz , and its employees from any and all liabilities, and/or the costs associated with any veterinary care, symptoms or discomfort of your pet related to or presenting after services rendered by Mutt Kuttz. The pursuit of any veterinary, behavioral or rehabilitation services, and the payment there of, is the sole responsibility of the pet owner. Additionally, you agree to relieve Mutt Kuttz and its employees from any and all liability associated with damages to person, pet or property by or relating to services rendered or service equipment used by Mutt Kuttz and its employees.

 

PRICING: All haircut and bath prices are based on a 4-6 week maintenance grooming schedule and in good coat condition. Price increase will apply for length of time since last grooming, matting, fleas, behavior, etc. Mutt Kuttz reserves the right to alter pricing as needed to account for cost of quality increases without notice. Additionally, Mutt Kuttz reserves the right to charge additional fees for services we consider over and above the norm covered by our standard rates.

 

PAYMENT: Mutt Kuttz accepts Cash, Checks (established clients only), Visa, MasterCard, American Express and Discover. A $5 fee will be applied to over the phone payments. ALL services must be paid for by or at the time of pick up. Failure to pay will result in holding of the pet until services are paid in full. Overnight charges WILL apply if payment is not received by the end of the business day (please see overnight/abandoned pet section)

 

CANCELLATION: A 48-hour notice must be given when cancelling an appointment. This allows Mutt Kuttz adequate time to fill the appointment slot. This includes appointments booked the day of. Failure to provide a 48-hour notice will be considered a no call no show and no-show policies as outlined below will be in effect.

 

NO CALL NO SHOW: No call no show fees will be charged as outlined below. All no call no show fees must be paid PRIOR to booking the next appointment. Emergency circumstances will be taken into consideration at the discretion of Mutt Kuttz staff.  Although Mutt Kuttz may send appointment reminders, it is the owner’s responsibility to remember appointments. Mutt Kuttz is NOT responsible for reminding customers of appointments. Please understand that our groomers are paid on commission, no call no show appointments directly affect their paychecks as well as the facility. NO CALL NO SHOW FEES: First offense: $20 fee PER pet will be charged. Second offense: 50% of the booked services cost will be charged (applies to ALL scheduled pets) Third and beyond offenses: 100% of the booked service cost will be charged (applies to ALL scheduled pets)

 

 

LATE PICKUP: After hours pick up will be charged $1 per minute after closing time up to 45 min after closing time. Any pet left more than 45 minutes after closing will be considered an overnight/abandoned pet and will be charged as such. (Closing time is 4:00pm Monday-Thursday unless otherwise posted)

 

OVERNIGHT/ABANDONED PETS: Mutt Kuttz is NOT a boarding facility. All pets must be picked up by designated closing time (4:00 pm Monday-Thursday) or the account will be charged $80 per night. If a pet is not called for/picked up within 10 days, Mutt kuttz will rehome the pet as they see fit within 10 days of this period. ONE notification will be sent during this time by certified mail. No other notice will be provided.

 

FLEAS AND TICKS: If pet(s) are found to have fleas at any point during the grooming process, pet(s) will be treated and accounts will be charged accordingly. Mutt Kuttz uses a flea and tick shampoo designated to eradicate any live fleas, but is NOT intended to be used as a preventative so the owner will need to pair it with a preventative to fully rid the pet of fleas. If the pet is so flea/tick ridden that it poses a threat to the pet’s health, Mutt Kuttz reserves the right to refuse service.

 

EXPRESS GROOMS: Mutt Kuttz treats all grooms as “express grooms” with no extra charge. If you are unable to pick up your pet within 30 minutes after the groom is completed please let us know ahead of time as we have limited kennel space.

 

COUPONS: Referral coupons/offers cannot be combined or stacked with other offers. Coupons cannot be used on “special pricing” clients/grooms. All coupons/offers are subject to change without notice. Mutt Kuttz is not responsible for missed coupons due to forgetting to mention them or present them. NO refunds will be provided for missed or forgotten coupons.

 

BELONGINGS: Mutt Kuttz is not responsible for items lost, broken, chewed etc. as items are left at the risk of the owner. This includes but is not limited to leashes, collars, bedding etc. Additionally, the owner agrees to be responsible for any property damages by the pet.

 

INJURED/ILL PETS: If a pet is hurt or becomes ill, Mutt Kuttz has permission to call or take the pet to the veterinarian of our choice if the pet’s veterinarian is inaccessible, or give other advisable attention, within our discretion and judgment. Such expenses will be paid promptly by the owner or responsible party unless injury is the direct result of negligence of Mutt Kuttz. We will not be held responsible for any sickness or injury caused by the pet to itself during or after grooming. Mutt Kuttz is not responsible for any accidental death of pets due to pre-existing health conditions or natural disasters (fire, storm, flood, etc.) Mutt Kuttz will not be held responsible for clipper irritation and/or minor nicks resulting from grooming of matted, neglected coats OR for irritation caused by removing coat from pets possessing mild to severe skin allergy. Mutt Kuttz will not be held responsible for clipper irritation resulting from shaving pets, the owner takes responsibility for the inherent risk of skin irritation when/where clippers are used. Mutt Kuttz will not be held responsible for skin reactions from shampoo. Nor will we be held responsible for stress effects grooming may have upon a pet. Mutt Kuttz must be notified within 48 hours of any claims by the owner regarding compensation for medical attention sought as a result of alleged negligence on our part. In the event Mutt Kuttz disputes the owners claim, we will personally consult the attending veterinarian prior to settling any claim.

 

 

PHOTOGRAPHS: By allowing Mutt Kuttz to care for pets, the owner authorizes Mutt Kuttz to take and utilize pet photographs. This includes use for advertising, Facebook, Twitter, Flyers, coupons, website, etc. No notice or compensation will be given for use of photographs.

 

Please Note: Mutt Kuttz reserves the right to refuse/deny service to any pet posing an unnecessary risk to themselves, the facility, or our groomers.  Mutt Kuttz also reserves the right to refuse service to customers whose pet may pose a threat to us or the other pets left in our care, whether it be due to an aggression problem, a health problem, a parasite problem (other than fleas) or heavily matted. Mutt Kuttz reserves the right to change or modify our policies without prior notice, any future updates to our policies will be updated on our website at muttkuttz.com under tab marked GROOMING RELEASE FORM AND POLICIES. It is the sole responsibility of each client to check this site for updates or to check in the store for a copy of changes to this GROOMING RELEASE FORM. Matted Dog Release Form

Matted coats can cause a variety of skin & health problems. Matted fur does not allow for air circulation to the skin, causing hot spots, bacterial and fungal infections. Stickers as well as fleas, ticks, maggots and other parasites may be lurking in the coat causing further skin infections. Matted fur also pulls and binds, causing pain to your pet when they move or lay on mats. The skin underneath is usually raw and inflamed or bruised. Matted coats will not dry properly and can lead to rotting fur and skin. X______

 

Due to the coat condition of my dog, Mutt Kuttz Doggie Spa has determined that the only humane thing to do is to shave your dog’s coat down. This is a very short clip to remove all the matting allowing the skin to breathe again. After the groom your dog may show signs of skin irritations, sanitary infections due to feces and urine being trapped against your pets skin, eye infections or other eye issues due to eye “gunk” and moisture being trapped so close the eyes and delicate skin near the eyes, bruising or sores due to matting and wet/dirty coat. Some dogs may itch, self-mutilate (scratch or bite themselves due to irritation the matting caused and cause bleeding) or have behavior changes after the groom. Ear hematomas can form due to the matting cutting off circulation to the ears. Some of these issues may require vet care at owner's expense. X________

 

The matted hair rests tightly against the skin and because of this your dog may develop clipper irritation or be nicked or cut because the groomer has to work so closely to the skin to remove the matted coat. Our groomers are very careful, but the possibility of injury exists due to the lack of proper grooming causing the matting to occur. Understanding all of these risks, and understanding that other health issues may arise Mutt Kuttz Doggie Spa is authorized to proceed with the de-matting process. X________

 

I hereby release Mutt Kuttz Doggie Spa from any liability associated with the above-mentioned process and any and all injuries or medical problems that may be uncovered and/or occur during the stripping and de-matting process. Should the pet need veterinarian care after or during the process of dematting, the owner agrees to pay any and all veterinarian fees. I will not slander nor libel Mutt Kuttz Doggie Spa for shaving my dog in an attempt to help make them feel better, or for any medical conditions that may arise. I understand it is my duty as a pet owner to provide proper grooming for my pet to avoid the matting in the future. I understand pictures will be taken to protect Mutt Kuttz Doggie Spa and that additional fees may be added depending on the time it takes to safely demat my pet. X________
